  • Application for New passport in Lieu of Expired or Exhausted passport

    All applicants should submit:

    1. Old passport.
    2. Original Pakistan National Identity Card along with a photocopy.
    3. Four current passport size (2X2, Front Face) latest photographs.
    4. Application Forms: "A", "T" and Memorandum.

    Note: Applicants under 18 years of age should submit an original RG-II (Registration From "B") along with a photocopy. In the absence of either National Identity Card or Registration Form RG-II, a restricted passport of one (1) year will be issued. Also, a Pakistan National Identity card with photograph is required for all women, otherwise restricted passport will be issued for one year.

  • Issuance of passport in lieu of lost passport

    1. Complete (FORM -A)
    2. Complete (FORM -D) with one notarized photograph.
    3. Complete (Memorandum)
    4. Complete (Report of lost passport)
    5. 6 Current Photographs
    6. Original national ID card and photocopy of I.D card
    7. Renewal fee (Please check fee structure)
    8. Copy of lost passport (if available)
    9. Police Report
    10. Self addressed prepaid envelope if sending by mail.(Use USPS Express Mail Only for urgent items. We Do Receive envelopes through FedEx, UPS, DHL and Airborne Express, but we Cannot Send return envelopes through them. Kindly ensure that return envelopes are of USPS only)
    Note: Issuance of passport in such cases is subject to verification from original issuing authority

  • Renewal of Expired Passport Requirements

    1. Complete Form-A
    2. 4 Current Photographs
    3. Original national ID card and photocopy of I.D card
    4. Renewal fee
    5. Expired passport
    6. Proof of resident in US (Notarized copy of green card/driving license)
    7. Applicant under 18 years of age should provide registration FORM -B of NIC.
    8. Self addressed prepaid envelope if sending by mail.(Use USPS Express Mail Only for urgent items. We Do Receive envelopes through FedEx, UPS, DHL and Airborne Express, but we Cannot Send return envelopes through them. Kindly ensure that return envelopes are of USPS only)
    Note: Passport with a one-year validity will be issued in the absence of NICOP, Form-'B'. The applicant will have to submit his NICOP application at the same time.

    Children will not be endorsed on Parent's Passports and should apply for separate passports. Children born in USA should attach following with their Passport application:

    1. S-1 Registration form
    2. Form-A
    3. Notarized copy of father's Pakistani passport or US passport (of Pakistani Origin)
    4. 4 photographs
    5. Renewal Fee

  • Endorsements/Amendments in Passport

    1. Complete (FORM -C)
    2. Original national ID card (Copy if original is not available).
    3. Two photographs of child - endorsements in passport ( Note): It is advisable that the children may be issued separate passport.
    4. Endorsment/Amendment fee $6 (Money order) $12 for urgent.
    5. Self addressed prepaid envelope if sending by mail.(Use USPS Express Mail Only for urgent items. We Do Receive envelopes through FedEx, UPS, DHL and Airborne Express, but we Cannot Send return envelopes through them. Kindly ensure that return envelopes are of USPS only)

    Note: No fee charged for extension of limited validity passport. If NICOP/CNIC is shown, passport of limited validity is extended for free. Please note that limited validity Passport will only be extended on production of NICOP/Form RG-II (Form B) is required in case of children below 18 years.
