• Adults, Complete Form PP.11
• For children 16 years or younger, complete Form PP.21

1. Your current passport.
2. Two passport size pictures (full face showing both ears).
3. A money order of:
   • US$50.00 for a 32-page passport
   • US$70.00 for a 48- page passport
   • US$165.00 for mutilated passport
   • US$40.00 for C.I.N.
   • US$25.00 for Child Endorsement.
   • US$15.00 for amendment of profession and change of name.
4. A self-addressed stamped envelope for the return of your passport(s). The envelop should have enough postage for Priority Mail, Express Mail, FedEx, UPS, Airborne Express or DHL.

Lost Passport :

If you have lost your Kenyan passport,

Complete Form PP.11

Submit the form with the following:

1. Two passport size pictures (full face showing both ears).
2. A money order for US$200.00.
3. A copy of your birth certificate.
4. A copy of the police report stating how you lost your passport.
5. A copy of Form I-20 if you are a student or a copy of green card if you are a permanent resident.
6. A self-explanatory letter of how your passport got lost.
7. Duly completed questionnaire Particulars of a Lost Passport.
8. A self-addressed stamped envelope for the return of your passport(s). The envelope should have enough postage for Priority Mail, Express Mail, FedEx, UPS, Airborne Express or DHL.
9. Your full address.

Passport Renewal :

If you have lost your Kenyan passport,

Complete Form PP.11

Submit the form with the following:

1. Your current passport.
2. Money order of:
   • US$25.00 for a 32-page passport
   • US$40.00 for a 48-page passport
   • US$50.00 for renewal of Diplomatic passport
   • US$25.00 for Child Endorsement
   • US$15.00 for amendment on profession or change of name
3. A self-addressed stamped envelope for the return of your passport(s). The envelope should have enough postage for Priority Mail, Express Mail, FedEx, UPS, Airborne Express or DHL.
