New Bangladesh Passport:

04(Four) copies of passport size photograph along with the application form. One photograph should be duly attested by any professional person with proper seal.

For a child's separate passport, endorsement of that particular child's name in his/her mother's Bangladesh passport is a precondition.

Renewal of Bangladesh passport:

• Fill-in form.
• Two passport size recent photographs
• Fee
• Self-addressed pre-paid registered envelope or pre-paid return courier.

Lost passport in Bangladesh:

• Fill-in Passport application form.
• Four passport size recent photographs - duly attested.
• Police Report.
• Photocopy of lost passport and SSC Certificate (Original) (Where applicable) or Birth certificate.
• A certificate from residence owner with utility bills or own house / Tenenacy agreement. See the certificate to be filled-in by the Landlord/relative of the applicant.

Endorsements:

1. Endorsement of Child's Name:

   Child's detail Birth Certificate where parents name are included to be submitted along with mother's Bangladesh Passport and child's 02 copy of photograph.

2. Endorsement of Husband's name:

   Registered Marriage Certificate should be enclosed.

3. Corrections:

   Affidavit in support of correction duly, legalized by solicitor to be enclosed.
